Procedure for Absence Justification due to illness

Step 1 – Get a medical certificate

Step 2 - Submit to the Doctor on campus (block 19)

Step 3 - Submit the signed Absence Justification form to course instructor (one form for each course)

Step 4 – If approved, arrange with instructor for all make-up of missed assignments/quizzes with new deadlines.
